In an continuing effort to provide a safe environment for our players:

Regarding a required CONFIDENTIAL SCREENING SHEET, it is the policy of the Wheatfield Blades that, prior to exercising their duties, every Board member, Coach, Assistant Coach, Team Manager or Rep, or anyone else who is on the ice or in the locker room with any Wheatfield Blades team at any WBHA activity, shall first fill out such SHEET and file same with the designated WBHA official responsible for such forms.

These screening sheets will be kept strictly confidential by the WBHA and will only be viewed by the WBHA President and/or his designated official responsible for these forms.  They shall be supplied only to the designated official of NYSAHA and/or USA Hockey as per their requirements, and shall be used solely for the purpose intended by these associations.  Any unauthorized handling of such forms, or any dissemination of the information contained on such forms shall be a serious breach of WBHA rules and will be dealt with accordingly."
